Receive student ID card
Purchase ASB ($40) sticker (required for sports participation)
Yearbooks
Century Club cards ($125) will also be available for purchase.
You will be cleared to participate in practices once all documentation has been completed and turned into the front office at the high school.
IMPORTANT: Forms must be delivered to the school office the day of registration. DO NOT mail or give them to coaches. All forms must be cleared through the office.
Please bring the following items with you to the sports registration:
Interim Questionnaire & Travel/Permission Form - This form must be signed by both the student and parent/guardian. Please make sure all sections are completed entirely and signed before handing to office personnel. A copy of your current insurance card needs to be attached securely to the interim questionnaire.
Sports Medical Physical - All athletes must have a physical on file. All athletes entering 9 and 11 grades must have a physical dated after May 1, 2023. All transferring students must have a new physical.
